Community Living Exchange Collaborative at ILRU

The Community Living Exchange Collaborative is a four-year grant funded by the Centers for Medicare & Medicaid Services (CMS) in October 2001 to provide technical assistance with issues relating to system change in Community Living. The Collaborative Exchange Grant was jointly awarded to The Independent Living Research Utilization (ILRU) in Texas and The Rutgers Center for State Health Policy in New Jersey. CHANCE is partnering with ILRU to manage its activities on the Exchange Collaborative with specific emphasis on housing and nursing facility transition.
